停用 GLP-1 后为什么容易体重反弹:先保住肌肉,再谈长期维持
Why Weight Regain is So Common After GLP-1 Medications—and What Actually Helps to Avoid It
Weight loss medications have transformed the way many people lose weight. They’ve had the biggest impact of anything I’ve seen in my two decades working in the field. But to me, successful weight loss means long-term weight loss—and maintaining that weight loss after coming off medication can be challenging.
Research shows that weight regain is common after stopping GLP-1s. One landmark semaglutide trial found that people regained around two-thirds of the weight they had lost within a year of ending treatment.
Recently, my hairdresser told me he’d lost 42 pounds on Mounjaro, only to regain it after coming off it. He went back on the medication and admitted he now feels nervous about stopping again—and he’s far from alone.
The good news? If you’re on weight loss medication and don’t want to stay on it forever, maintaining your weight loss naturally is possible with the right habits and support.

Why does weight regain happen after GLP-1 medications?
Weight regain is common after many forms of dieting—not just after GLP-1 medication.
Many of my clients have lost weight on different diets over the years, only to regain it later. So why does this happen, and how can we prevent it?
First, mindset is key. Whether you’re losing weight naturally or with medication, one of the biggest mistakes is reaching your goal weight and thinking the work is done. It isn’t. Maintenance is the start of a whole new phase, and one that requires its own strategies.
Yes, you’ll usually be able to introduce a little more flexibility compared with the weight loss phase, but key nutrition and lifestyle habits still matter.
How to help maintain weight loss after GLP-1s—and avoid rapid weight gain
1. Prioritise protein and fiber
From a dietary perspective, I recommend continuing to structure meals in the same way I outline in my new book, The Nozempic Diet. That means prioritizing protein (aiming for around 30g per meal), eating moderate amounts of healthy fats such as olive oil, avocado, raw nuts, seeds, and including plenty of fiber-rich vegetables.
Once you’ve maintained a weight you’re happy with, you can reintroduce moderate portions of carbohydrates such as beans, lentils, chickpeas, oats, potatoes, and brown rice.
2. Keep ultra-processed foods to a minimum
I advise continuing to limit sugar, refined carbohydrates, and ultra-processed foods where possible. Many people find these foods less filling and more likely to trigger overeating once appetite returns after medication.
3. Don’t underestimate the role of sleep and stress
Sleep has a surprisingly significant impact on appetite and satiety. Aim for at least seven hours of quality sleep where possible.
Stress and emotional eating can also sabotage progress, making weight maintenance more difficult. It’s important to recognize when the urge to eat is being driven by emotion rather than genuine hunger.
Strategies such as spending time outdoors, movement, and breathing exercises can all help support nervous system regulation.
4. Aim for consistency, not perfection
Holidays, celebrations and festive periods can all affect our weight. The goal isn’t to maintain perfect habits all year round, but to avoid an “all or nothing” mentality where healthy routines disappear completely.
The magic happens when healthy habits become part of your everyday life. They become so ingrained that they’re simply what you do—and something you naturally return to after holidays or celebrations.
Why muscle matters after GLP-1 weight loss
Research shows that a significant proportion of the weight lost on GLP-1 medications is muscle.
While this may mean the number on the scales drops more quickly in the short term, it can make long-term weight maintenance more difficult.
Muscle is metabolically active, meaning the more muscle we have, the more calories we burn—even at rest. Losing muscle can reduce metabolic rate and contribute to the rapid weight regain many people experience after stopping medication.
If you’re currently taking a GLP-1, I strongly encourage you to prioritize protein intake and strength training throughout treatment. This can help preserve muscle mass during weight loss.
Appetite rebound is normal
Another major factor is the return of appetite. While some people find it comes back gradually, others say it returns with a vengeance.
GLP-1 medications typically reduce both appetite and the desire to eat. Once that changes, it can feel unsettling—but it is normal.
Understanding how to structure meals in a filling and satisfying way, alongside establishing regular mealtimes, becomes especially important.

Some experts recommend weighing yourself regularly after stopping GLP-1 medication to help spot gradual changes early – but small fluctuations are completely normal
Should you weigh yourself after stopping GLP-1s?
One thing I strongly recommend when maintaining weight loss is continuing to monitor your weight regularly—once a week works well for many people.
It can be tempting to avoid the scales, particularly if your weight starts creeping up slightly after a holiday or festive period, but staying aware of what’s happening can help you make small adjustments before things escalate further.
This isn’t about obsessing over the number on the scales—small fluctuations are completely normal. It’s about maintaining awareness and being proactive about managing your health and weight.
What not to do if you regain weight
If you find yourself regaining weight after stopping medication, try not to panic or compensate with extreme measures. Avoid skipping meals, overexercising, or crash dieting. Some fluctuation is completely normal, particularly as appetite returns and your body adjusts.
The goal is to establish a way of eating and living that is realistic, enjoyable, and sustainable long term.
Coming off GLP-1 medication can feel daunting, but with the right strategy, structure and support, maintaining your weight loss is absolutely possible.
